Community Assessment Survey
We are inviting you to participate in a survey that is being conducted by the Developmental Disabilities Institute at Wayne State University. This survey will help the Institute in developing its next 5-year plan.
To help in generating ideas, we have a brief survey for you to complete. The survey will take less than 10 minutes to complete. You can complete it on line, or you can download a paper version of the survey. Feel free to distribute this on line survey link, as well as paper copies of the survey, to anyone in Michigan who has a developmental disability, is a family member of a person with a developmental disability, or a service provider, advocate or policy maker.
