Survey Recruitment: Parents of a child 18 years-old or younger
Are you a parent of a child 18 years-old or younger? Whether you have a toddler or a teen, the fall season brings thoughts of a new school year. What are your hopes and worries for your child? Please take our brief, 5-minute survey about your child's health & wellbeing. Your identity and responses will remain private. You could win a $50 gift card, if you complete the survey by December 1, 2017. For every 20 participants, we will make a donation to the UNIDOS Disaster Relief Fund to Mexico and Puerto Rico.
